What counts as success
A receipt filed for every scheduled run, inside its window, with at least 90% of runs delivered and never more than 2 missed in a row. Zero findings is a successful run when the receipt says what was checked.

Report only confirmed, source-backed items from the last 7 days: CMS Special Focus Facility list changes, immediate jeopardy citations and fines over $50,000 at named nursing homes; state assisted living and home care agency license actions; state Medicaid and veterans programs that pay family caregivers, with rates and eligibility; respite, adult day and caregiver grant openings; dementia and Alzheimer treatment coverage and trial enrollment news from NIH and CMS; caregiver tax credit, leave and workplace rule changes; and elder financial abuse alerts. Prefer CMS, state health and aging agencies, VA, NIH and established aging reporting; ignore facility marketing and unverified reviews. Put the facility, state or program first in the title. Use high severity for an immediate jeopardy citation, a program paying $1,000 or more a month, or a grant closing within 30 days. Return no more than 6 findings. Push each as a finding with a title, a two-sentence summary, sections for what happened, who it affects, what to do, and the source link. If nothing qualifies, push nothing and submit the run receipt.
