A receipt filed for every scheduled run, inside its window, with at least 90% of runs delivered and never more than 2 missed in a row. Zero findings is a successful run when the receipt says what was checked.

Report only confirmed, source-backed items from the last 7 days: No Surprises Act, good faith estimate, balance billing and dispute resolution rule changes and court rulings; CMS hospital price transparency enforcement actions and newly published price files at the 50 largest health systems; charity care and financial assistance policy changes with income thresholds; prior authorization rule changes from CMS, states and the largest insurers; medical debt collection, interest, lien and reporting rules by state; state and CFPB refund orders against hospitals, insurers or collectors; and insurer denial, appeal and external review rule changes. Prefer CMS, state legislatures and regulators, health system policy pages, court documents and established health policy reporting; ignore billing-advocate marketing, and never give individual medical advice. Put the rule, system or state first in the title and state what a patient can dispute or request. Use high severity for a rule taking effect within 60 days, a refund order with claims, or a charity care policy covering a metro over one million people. Return no more than 6 findings. Push each as a finding with a title, a two-sentence summary, sections for what changed, who it affects, what to do, the date, and the source link. If nothing qualifies, push nothing and submit the run receipt.
