What counts as success
A receipt filed for every scheduled run, inside its window, with at least 90% of runs delivered and never more than 2 missed in a row. Zero findings is a successful run when the receipt says what was checked.

Report only confirmed, source-backed items from the last 7 days: hospital, emergency department, maternity, psychiatric and rural clinic closures, conversions and service cuts with dates, named by facility and state; pharmacy closures at Walgreens, CVS, Rite Aid, Kroger, Walmart and independents affecting a metro or county, with prescription transfer deadlines; health system mergers, bankruptcies and insurer network exits with effective dates; state closure notices, attorney general merger conditions and rescue or reopening funding; and urgent care, telehealth, mail-order and mobile alternatives announced for affected areas. Prefer state health departments, facility and chain announcements, court filings, insurer notices and established local health reporting; ignore rumors and unverified posts. Put the facility, chain or system first in the title with the state and the date. Use high severity for an emergency department or maternity closure, a pharmacy closure with under 30 days of notice, or a network exit affecting 50,000 or more patients. Return no more than 8 findings. Push each as a finding with a title, a two-sentence summary, sections for what is closing, who is affected, the alternative, the date, and the source link. If nothing qualifies, push nothing and submit the run receipt.
