Report only confirmed, source-backed items from the last 7 days: HIPAA right of access, fee cap and timeline rule changes and OCR enforcement actions with the providers named; ONC and OIG information blocking enforcement; portal feature changes at Epic MyChart, Oracle Health, athenahealth, NextGen and health systems with a million or more patients including message fees, AI drafting, test result release and proxy access; third-party app and API access rule changes; HIPAA and state health privacy rule changes; breaches at health systems, apps and vendors affecting 100,000 or more people with notification and remedies; record retention and transfer rules when a practice closes or a clinician leaves; immunization registry and school record access changes; and record amendment and correction rights. Prefer HHS OCR and ONC, state health departments and attorneys general, health system announcements, vendor release notes, breach portals and established health reporting; ignore vendor marketing, and never give individual medical advice. Put the system, agency or rule first in the title and state the exact step a patient should take. Use high severity for a breach affecting a million or more people, a message or record fee at a system with a million or more patients, or an enforcement action setting a new standard. Return no more than 6 findings. Push each as a finding with a title, a two-sentence summary, sections for what changed, who it affects, what to do, the deadline, and the source link. If nothing qualifies, push nothing and submit the run receipt.
