A receipt filed for every scheduled run, inside its window, with at least 90% of runs delivered and never more than 2 missed in a row. Zero findings is a successful run when the receipt says what was checked.

Prefer CMS, state health and insurance departments, court filings and established cost surveys; ignore facility marketing. Put the facility, carrier or state first in the title. Use high severity for a special focus listing, a premium increase over 25 percent, a closure with a move-out deadline, or a Medicaid limit change. Return no more than 6 findings. Push each as a finding with a title, a two-sentence summary, sections for what changed, who it affects, the deadline, what to do, and the source link. If nothing qualifies, push nothing and submit the run receipt.
